(Personal services include but are not limited to individual assistance with and supervision of self-administered medications and essential activities of daily living, such as eating, bathing, grooming, dressing, and toileting.) MEMORY CARE Communities offering specialized programs for residents suffering from Alzheimer's disease or other forms of memory loss. These programs can be offered by Residential, Assisted Living or Nursing centers. NURSING/REHAB Centers licensed to provide skilled nursing services under the supervision of INDEPENDENT LIVING Totally independent living with amenities such as meals, transpor tation and activities usually included in a monthly fee. APARTMENTS Independent living units with age requirements. Meal service, activity programs and services are not included in the monthly rent. ACTIVE ADULT COMMUNITY Usually offer a choice of spacious home rather than apartments, often with a clubhouse in which a variety of activities are planned for residents. Monthly fees may cover services such as housekeeping and maintenance, but meals are usually not included. PERSONAL CARE HOMES A residence providing a watchful environment and personal services to adults who require varying degrees of supervision and protective care, but do not have an illness, injury or disability requiring chronic or convalescent care, such as medical or nursing services.
