Amity Hospice is unique…because within 48-72 HOURS after a patient
is admitted into our care, an Amity Physician meets with every patient
and their family. During these first hours, we discuss a tailored plan of
actionable next steps, answer any questions and ease all fears as much
as is possible. From day one, a dedicated Case Manager, who is an RN,
will be there every step of the way.
We are also unique because for every patient of ours, we assign a
care team that supports our physician-directed care plan.
