Our caring team works together providing each
resident with an individualized plan of care that meets
their unique needs and maximizes their strengths to
facilitate returning home. We provide comprehensive
skilled services including IV therapy, PICC lines, Bi
PAPs, Trach Care, Special Wound Care, PT, OT and
ST. Our program also includes Hospice and Palliative
Care with an emphasis on Pain Management. We
provide continuity of care through our work with
managed care companies and our community
programs.
Participants in this program
receive individualized
nursing, medical and rehab
services that allow them to
maintain maximum indepen-
dence and remain in the comfort of their homes. Appli-
cations may be made after discharge from a hospital or
long term care facility, or directly from home. Medicare,
Medicaid, and private pay are accepted. Licensed in 7
counties and available to residents of Bronx, Brooklyn,
Manhattan, Nassau, Queens, Suffolk and Westchester
RiverSpring Home Care Services provide one-on-one
health care and supportive services to help maximize
independence in the comfort of ones' own home. We
can coordinate and provide a full range of health,
social, and environmental services tailored to your
needs. Our goal is to ensure independence and dignity
for those wanting to remain at home. By developing a
comprehensive customized plan of care, people are
able to live in the secure environment where they are
