Our caring team works together providing each
resident with an individualized plan of care that
meets their unique needs and maximizes their
strengths to facilitate returning home. We provide
comprehensive skilled services including IV therapy,
PICC lines, Bi PAPs, Trach Care, Special Wound
Care, PT, OT and ST. Our program also includes
Hospice and Palliative Care with an emphasis on
Pain Management. We provide continuity of care
through our work with managed care companies and
our community programs.
St. Johnland's 24-bed subacute unit offers intensive
rehabilitation services for our patients. Our skilled
physical, occupational and speech therapists work
one-to-one to assure a swift recovery. The expanded
rehab team of physicians, nurses, social workers and
other professionals provide every opportunity for
patients to regain their health. The unit offers a
private dining room, dedicated rehab suite and
garden terrace as well as extensive recreation
activities and programs in a beautiful woodland
setting.

seniors. We promote
independence and rapid
return to home after rehabilitation from a broad range
of surgical procedures, stroke, amputation, injuries and
illness. Patients receive state-of-the-art rehabilitation,
individualized treatment plans, and, if required,

care, private pay and many insurance plans accepted.
