Our specialized team of hospice professionals works with the patient, family
members and attending physician to develop a plan of care that will best
meet the patient's needs. An individualized plan of care may also include
other disciplines to ensure patient comfort.
Upon discharge from the hospital, our Transitional Care Team will be there
to educate the family and patient as they adjust to their new circumstances
to lessen the chance of unnecessary rehospitalization.
