Young, all phases of life are important. End of life is an opportunity for terminally ill persons and their families to celebrate their relationships and experiences, as well as coming to terms with life in general. C. C. Young Hospice works with the individual to help ensure each person is able to enjoy this time of develop ment. Through our qualified and supportive hospice staff we provide comfort, including pain management, clergy sup - port if desired, and nursing care. Keeping the individ ual comfortable and free of pain enables each individual to find acceptance and fulfillment in this final stage of life. Hospice is a philosophy of care, which enables patients and their families to make choices about care in life's final stage. Patient and family needs are met, using a team approach. Christian Care Hospice Team consist of physicians, nurses, social worker, therapists, dietician, chaplain, bereavement counselor, and volunteers. Our professionals schedule visits based on the needs of the patients. Hospice nurses are on call 24 hours a day, 7 days a week to answer questions and address any concerns with our patients. Our team understands life is still happening every day, even during one's most difficult circum stances. We believe the right care enables hospice patients to experi ence life with quality and dignity. Our team members are available 24/7. We are serious about doing the right thing and every member of our team is empow - ered to do whatever it takes to achieve it. Often it is the little things that matter the most. To us, that is what it means to have life with dignity. Elysian Hospice's (pronounced "e-lee-shun") mission is "to surround a terminally ill individual and their family with comfort and dignity through the process of death and bereave - ment." We are committed to providing a comprehen sive program of services that will optimize a person's quality of life when curative treatment is no longer possible. This is accomplished by providing physical, emotional and spiritual support for our patients and their families.
