This list was obtained from the New Jersey Department of Health and Senior Services - Division of Health Facilities Evaluation & Licensing website. Assisted Living Residence (ALR) is a residential facility licensed to provide apartment-style living and congregate dining, Comprehensive Personal Care Home (CPC) is a facility licensed to provide room and board. Assisted Living Program (ALP) is a package of assisted living services including nursing and personal care to be provided by a licensed agency. Alternate Family Care (AFAC) is a service provided under contractual agreement whereby no more than three persons receive room and board, personal care and other health services in the home of an unrelated individual.
