TYPES OF SENIOR LIVING AND CARE APARTMENTS Independent living units with age requirements. Meal service, activity programs and services usually are not included in the monthly rent. AFFORDABLE SENIOR HOUSING HUD 202 Program that offers rental assistance for seniors who meet the requirements of the federal program. CCRC Continuing Care Retirement Community. Combine independent living, assisted living and nursing in a single setting. Normally requiring an entry fee, offering a living unit, meals, and health care, up to the nursing level. RETIREMENT Independent living with amenities such as meals, transportation and activities usually included in a monthly fee. ASSISTED LIVING Assisted living facilities provide individualized health and personal care assistance in a homelike setting with an emphasis on personal dignity, autonomy, independence and privacy. Facilities can be large apartment-like settings or private residences. Services include meals, bathing, dressing, toileting and administering or supervising medication. RESIDENTIAL CARE Usually single family homes licensed to provide assistance with medications, bathing and dressing. MEMORY CARE Communities offering specialized programs for residents suffering from Alzheimer's disease or other forms of memory loss. These programs can be offered by Residential, Assisted Living or Nursing centers. NURSING/REHAB The Nursing Facility program provides institutional care to Medicaid recipients whose medical condition regularly requires the skills of licensed nurses. The nursing facility must provide for the total medical, social and psychological needs of each client, including room and board, social services, over-the-counter drugs, medical supplies and equipment, and personal needs items. HOME CARE Home Health Agencies provide a wide range of health and social services delivered at home to persons recovering from an illness or injury, or persons who are elderly, disabled and/or chronically ill. Home Health Agencies provide "skilled services" such as nursing, social services, and therapeutic treatments (physical, speech, occupational therapy). PERSONAL ASSISTANCE SERVICES This program contracts with licensed agencies to provide personal assistance services to the mentally competent clients with physical disabilities who are willing to supervise their attendant or have someone who can supervise the attendant. SITTER SERVICE Services of sitters, aides or private duty nurses or therapists in the home, hospital or residential center. May include personal care assistance, grooming, medication supervision, light housekeeping, transportation, nursing care or therapy. DAY CARE Licensed Day Activity and Health Services facilities provide daytime services, up to 10 hours per day, Monday through Friday, to individuals residing in the community. CARE MANAGEMENT Offer advisory services addressing a wide range of senior issues, such as selecting a senior residence, choosing in-home care providers, and various financial options. Typically care managers evaluate a senior's situation with regard to health needs, housing choices and financial needs and then provide a recommended care plan activities. HOSPICE A program of palliative care consisting of medical, social and support services provided to persons with a terminal illness and a physician's prognosis of six months or less to live. PRODUCTS & SERVICES Financial, health-related and other products and services designed to meet the varying needs of mature adults. 8
